    • Original Birth Certificate (with a raised seal) OR a valid US Passport. No photocopies are accepted by the MVC.
    • Original Social Security Card.
    • Proof of Written Test Completion (the blue card from high school, if applicable).
    • A $10 Check or Money Order made payable to "NJ MVC" (the MVC does not accept cash for permits).
    • A completed and signed MVC Permit Application Form (provided by the driving school).
